Programs of All-Inclusive Care for the Elderly (PACE)

The objective of PACE is to provide a range of integrated, preventative, acute, and long-term care services to manage the often complex medical, functional, and social needs of older adults. PACE was created to provide clients, family, caregivers, and professional healthcare providers, the flexibility to meet a person’s health and everyday needs, whilst continuing to live safely in the community. Ultimately, PACE seeks to maximize the dignity and unique needs of older adults and their immediate family unit. The Balanced Budget Act of 1997 established PACE as a permanent entity within the Medicare program, enabling states to provide PACE services to Medicaid beneficiaries as a state plan option. Operationally, the PACE program is unique as it is a three-way partnership between the Federal government, the State, and the PACE organization.

Interdisciplinary Teams

This is all possible because PACE employs interdisciplinary teams at the core of the program to exchange information & solve problems as the needs of each participating individual evolves.

Primary Care

Participants then have regular access to doctors and other primary care professionals who know them and specialize in caring for older adults.

Improved Health

Participants enjoy a recognizably improved health status, quality of life, lower mortality rates, increased choice in their time management, and greater confidence in dealing with life’s challenges.

Financial Model

The PACE financing model combines payments from Medicare & Medicaid or private pay sources into one flat-rate payment to provide the entire range of healthcare services, including payment for hospital care.

Transportation

PACE has an integrated transportation system that helps participants to live as independently as possible in the community while having access to support services, medical specialists, therapies, and other medical care sources when required.
