Autumnwood Care Center has offered premier quality skilled nursing and rehabilitation services since 1977. Originally known as Tiffin Geriatric Center, Volunteers of America purchased Autumnwood Care Center in 1979. We care for individuals with chronic diseases and for those recovering from injury or illness. Our residents receive 24 hour care from well trained, dedicated professional staff. Because we have the ability to coordinate complex medical care regimens, residents receive prompt, attentive care within our modern and home like setting. We recognize that individuals have unique physical, emotional and spiritual needs. Our staff understands that the living environment influences health and mental outlook of our residents. Families and friends are welcome at any time and are encouraged to be involved with their loved one's care and activities. Autumnwood Care Center is a 128 bed skilled care facility located at the east edge of Tiffin, Ohio on State Route 18. It's scenic rural setting provides the beauty of being in the country and at the same time the convenience of being near shopping, entertainment and recreational opportunities. We are a one floor facility with large private rooms as well as many shared rooms. The facility was built in 1977 and has undergone several major renovations including recent upgrades in the Therapy Department and remodeling and redecorating of the magnificent sunroom that is used for resident activities as well as community meetings. Several beautifully landscaped courtyards and fountain features add a dimension of excitement for those who enjoy an outdoor oasis.
